Resumen

Introducción: La anafilaxia perioperatoria es una emergencia tiempo-dependiente y potencialmente fatal. Su presentación puede estar enmascarada por la anestesia general y los campos quirúrgicos, por lo que la hipotensión refractaria, el broncoespasmo y/o el colapso cardiovascular suelen ser las pistas iniciales. El diagnóstico etiológico requiere un enfoque sistemático que combine criterios clínicos, medición seriada de triptasa y evaluación alergológica diferida. Reporte de caso: Se describe el abordaje diagnóstico-terapéutico de una reacción de hipersensibilidad inmediata durante anestesia general en un paciente de 35 años, con compromiso hemodinámico y respiratorio, que respondió a adrenalina intravenosa titulada, expansión con cristaloides y medidas de soporte. La triptasa sérica aguda fue elevada con normalización posterior, apoyando activación mastocitaria. La investigación alergológica posterior permitió identificar el probable agente desencadenante y establecer recomendaciones para anestesias futuras. Conclusiones: El reconocimiento temprano, la administración precoz de adrenalina y la reposición de volumen son determinantes del pronóstico. La toma oportuna de triptasa y la derivación a alergología permiten confirmar el mecanismo, definir el desencadenante y reducir el riesgo de recurrencia.

Abstract

Background: Perioperative anaphylaxis is a time-critical, potentially fatal emergency. Because general anesthesia and surgical drapes may mask cutaneous signs, refractory hypotension, bronchospasm and/or cardiovascular collapse often become the first clues. Aetiological diagnosis requires a structured approach combining clinical features, serial tryptase measurement and delayed allergy testing. Case report: We describe the diagnostic and therapeutic approach to an immediate hypersensitivity reaction during general anesthesia in a 35-year-old patient, with respiratory and hemodynamic compromise, successfully managed with titrated intravenous epinephrine, crystalloid resuscitation and supportive care. Acute serum tryptase was elevated and later normalized, supporting mast cell activation. Subsequent allergy work-up identified the most likely trigger and informed recommendations for future anesthesia. Conclusions: Early recognition, prompt epinephrine administration and adequate volume replacement are key outcome determinants. Timely tryptase sampling and referral to allergy specialists enable mechanism confirmation, trigger identification and recurrence prevention.
