ERITEMA MULTIFORME RECURRENTE ASOCIADO A HERPES SIMPLE CON COMPROMISO MUCOCUTÁNEO: DIAGNÓSTICO CLÍNICO, TERAPIA ANTIVIRAL Y ENFOQUE RECONSTRUCTIVO-ESTÉTICO

Resumen

Introducción: El eritema multiforme recurrente asociado a herpes simple representa una entidad mucocutánea inmunomediada, clínicamente desafiante por su tendencia a recurrir, su dolor, el compromiso oral y perioral, y su potencial impacto funcional y estético. Aunque el diagnóstico suele ser clínico, la diferenciación frente a síndrome de Stevens-Johnson, necrólisis epidérmica tóxica, reacción fija medicamentosa y mucositis infecciosa es esencial. Objetivo: Sintetizar la evidencia disponible sobre diagnóstico clínico, fisiopatología asociada a HSV, terapia antiviral supresiva y papel complementario de Cirugía Plástica en el cuidado mucocutáneo y la prevención de secuelas reconstructivo-estéticas. Métodos: Se estructuró una revisión sistematizada con lineamientos PRISMA 2020. Se incluyeron revisiones, series clínicas, ensayos y artículos de referencia sobre eritema multiforme recurrente, HSV, antivirales y manejo de cicatrices. Resultados: La evidencia identifica al HSV como desencadenante central de recurrencias; el ensayo controlado clásico de aciclovir continuo mostró reducción significativa de episodios. El manejo agudo se basa en soporte, analgesia, cuidado local, hidratación, control de infección desencadenante y vigilancia oftalmológica cuando existe compromiso ocular. El enfoque reconstructivo-estético no sustituye al manejo dermatológico/infeccioso, pero agrega valor en lesiones extensas de labios, mucosa oral, región perioral y áreas expuestas mediante curaciones, protección de barrera, prevención de sobreinfección, educación cicatricial y evaluación de secuelas. Conclusiones: El abordaje ideal es multidisciplinario. En casos recurrentes relacionados con HSV, la profilaxis antiviral continua debe considerarse individualmente; Cirugía Plástica puede contribuir a preservar función, estética y calidad de vida. 1-8,10-12,23-25.

Abstract

Background: Recurrent herpes simplex virus-associated erythema multiforme is an immune-mediated mucocutaneous disorder characterized by painful relapses, oral and perioral involvement, and functional and esthetic burden. Clinical diagnosis and careful distinction from Stevens-Johnson syndrome, toxic epidermal necrolysis, fixed drug eruption, and infectious mucositis are essential. Objective: To synthesize evidence on clinical diagnosis, HSV-related pathogenesis, suppressive antiviral therapy, and the complementary role of plastic surgery in mucocutaneous care and prevention of reconstructive-esthetic sequelae. Methods: A PRISMA 2020-oriented structured review was prepared, including reviews, clinical series, trials, and key articles on recurrent erythema multiforme, HSV, antivirals, wound care, and scar management. Results: HSV is a major trigger of recurrent disease. Continuous acyclovir prophylaxis has the strongest clinical trial evidence for reducing relapses. Acute management relies on symptomatic care, analgesia, topical therapy, hydration, etiologic control, and ophthalmologic evaluation when ocular involvement is suspected. Plastic surgery does not replace dermatology or infectious disease care but may support lip, oral, perioral, and exposed-skin lesions through advanced wound care, barrier protection, prevention of secondary infection, scar counseling, and assessment of functional or esthetic sequelae. Conclusions: A multidisciplinary approach is recommended. Continuous antiviral prophylaxis should be individualized in HSV-associated recurrent cases, while plastic surgery may help preserve function, esthetics, and quality of life. 1-8,10-12,23-25.
