Resumen

La insuficiencia cardíaca con fracción de eyección preservada (ICFEp) representa aproximadamente la mitad de los casos de insuficiencia cardíaca y su prevalencia va en aumento debido al envejecimiento poblacional y a la epidemia de obesidad y trastornos metabólicos (1) (2). Se caracteriza por signos y síntomas de congestión a pesar de una fracción de eyección normal, con disfunción diastólica y alteraciones extracardíacas. En la última década se han producido avances importantes en el diagnóstico y tratamiento de la ICFEp. En el diagnóstico, además de la ecocardiografía Doppler tradicional, se emplean técnicas avanzadas como el strain miocárdico longitudinal global y scores clínico-ecocardiográficos (H_2FPEF y HFA-PEFF) para mejorar la identificación de ICFEp en pacientes con disnea de etiología dudosa. Los péptidos natriuréticos siguen siendo biomarcadores clave, aunque su interpretación requiere cautela en pacientes con obesidad. En el tratamiento, si bien el control de comorbilidades y el uso de diuréticos para aliviar síntomas siguen siendo pilares, nuevas terapias han mostrado beneficio clínico. En particular, los inhibidores de SGLT2 reducen hospitalizaciones por IC en ICFEp (11) (12) y se recomiendan (clase IIa), mientras que el sacubitril/valsartán (ARNI) y los antagonistas de los receptores de mineralocorticoides ofrecen beneficios modestos en subgrupos y tienen recomendaciones clase IIb. La ICFEp en pacientes con obesidad constituye un fenotipo prevalente y desafiante, que requiere un abordaje integral con énfasis en la reducción ponderal. En adultos mayores, la ICFEp es frecuente y a menudo coexiste con fragilidad y etiologías específicas como la amiloidosis transtirretina, requiriendo un manejo individualizado. A continuación, se revisan de forma crítica los avances recientes en el diagnóstico y tratamiento de ICFEp, con foco en la obesidad y el adulto mayor, a la luz de la evidencia actual.

Abstract

Heart failure with preserved ejection fraction (HFpEF) accounts for roughly half of heart failure cases and its prevalence is rising due to an aging population and increasing rates of obesity and metabolic disorders (1) (2). HFpEF is characterized by signs and symptoms of congestion despite normal ejection fraction, resulting from diastolic dysfunction and extracardiac abnormalities. Over the past decade, important advances have occurred in HFpEF diagnosis and treatment. In diagnosis, beyond traditional Doppler echocardiography, advanced imaging such as global longitudinal strain and clinical-echo scores (H_2FPEF and HFA-PEFF) have improved identification of HFpEF among patients with unexplained dyspnea. Natriuretic peptides remain key biomarkers, though their interpretation requires caution in obese patients. In treatment, while control of comorbidities and diuretic use for symptom relief remain central, new therapies have shown clinical benefits. In particular, SGLT2 inhibitors reduce heart failure hospitalizations in HFpEF (11) (12) and are now recommended (class IIa), whereas sacubitril/valsartan (ARNI) and mineralocorticoid receptor antagonists confer modest benefits in subgroups and carry class IIb recommendations. HFpEF in obese patients represents a prevalent and challenging phenotype requiring comprehensive management focused on weight reduction. In older adults, HFpEF is common and often coexists with frailty and specific etiologies such as transthyretin amyloidosis, demanding individualized management. Here we critically review recent advances in HFpEF diagnosis and treatment, with an emphasis on obesity and older adults, in light of current evidence.
