MARPE VERSUS SARPE IN MAXILLARY TRANSVERSE DEFICIENCY: SYSTEMATIC REVIEW OF SKELETAL, DENTOALVEOLAR CHANGES AND CLINICAL SAFETY

Abstract

Introduction: Maxillary transverse deficiency in post-peak adolescents and adults is therapeutically challenging due to progressive maturation of the midpalatal and circummaxillary sutures. Miniscrew- or microimplant-assisted rapid palatal expansion (MARPE) has emerged as a less invasive alternative to surgically assisted rapid palatal expansion (SARPE). Objective: To compare skeletal, dentoalveolar, nasomaxillary, stability and safety outcomes of MARPE versus SARPE in patients with maxillary transverse deficiency. Methods: A systematic review was conducted according to PRISMA 2020. PubMed/MEDLINE, Scopus, Web of Science, Embase, Cochrane Library and manual reference searching were examined up to June 18, 2026. Twenty-five clinical, tomographic or comparative studies assessing MARPE/MSE/MARME, SARPE/SARME or minimally invasive hybrid techniques were included. Results: Direct comparative evidence was limited. The main comparative study reported greater skeletal transverse expansion at the midface and basal maxilla with MARPE, whereas SARPE produced greater dentoalveolar expansion and buccal inclination. MARPE showed favorable outcomes in post-peak adolescents and young adults, although success decreased with age and sutural maturation. SARPE remained relevant for severe adult discrepancies but involved higher morbidity. Conclusion: MARPE is an effective and less invasive option in selected patients, while SARPE remains important for severe cases or when non-surgical expansion is likely to fail.
