INSTITUTIONAL GOVERNANCE STRATEGIES FOR IMPROVING THE PUBLIC MANAGEMENT OF HEALTH SERVICES IN THE HOSPITALS OF THE CITY OF PORTOVIEJO

Abstract
The study addresses the importance of strengthening institutional governance in the hospital setting as a key element to improve the public management of health services in the city of Portoviejo. It is based on the recognition that hospitals face challenges related to resource optimization, strategic planning, and timely care for the population. The main objective was to analyze institutional governance strategies that influence the improvement of public hospital management during the year 2024. A qualitative approach was used under a positivist paradigm, with a non-experimental and cross-sectional design. Data collection was carried out through semi-structured interviews with hospital directors, allowing for an understanding of their experiences, challenges, and proposals. The results reveal the need to implement comprehensive strategies such as administrative and financial decentralization, strengthening the first level of care, digitizing processes through a unified electronic medical record, and adopting budgets based on services provided and outcomes. The study highlights the importance of citizen participation, transparency, accountability, and continuous staff training as essential elements to consolidate strong governance. These strategies will optimize resource allocation, improve operational efficiency, and increase public trust in health services. In conclusion, an integrated plan that combines strategic leadership, a data-driven culture, digitalization, and social participation can transform public hospitals into more approachable and accountable institutions, laying the groundwork for a replicable institutional governance model at the national level, aimed at the sustainability of the hospital system.
