BRONCOESPASMO SEVERO DURANTE LA INDUCCIÓN ANESTÉSICA: MANEJO INMEDIATO, DIAGNÓSTICO DIFERENCIAL Y DESENLACE. REPORTE DE CASO

Resumen

El broncoespasmo perioperatorio constituye una urgencia capaz de generar hipoxemia, hipercapnia, incremento abrupto de presiones de la vía aérea e inestabilidad hemodinámica, especialmente durante la inducción e instrumentación de la vía aérea. La etiología es heterogénea e incluye causas mecánicas (obstrucción del tubo o del circuito), intubación endobronquial, broncoaspiración, neumotórax a tensión, edema agudo de pulmón y broncoconstricción por hiperreactividad o por anafilaxia perioperatoria. Se presenta el caso de un paciente masculino de 56 años, programado para colecistectomía laparoscópica bajo anestesia general, que desarrolló broncoespasmo severo inmediatamente después de la intubación orotraqueal, con aumento marcado de la presión pico, caída del volumen corriente, desaturación y capnografía con morfología en "aleta de tiburón". Concomitantemente presentó hipotensión sin lesiones cutáneas evidentes, por lo que se consideró anafilaxia como diagnóstico de alta prioridad. El manejo inicial incluyó oxígeno al 100%, ventilación manual para evaluar complianza, verificación sistemática de causas mecánicas, profundización anestésica, broncodilatadores inhalados y soporte hemodinámico. Ante la respuesta incompleta y el compromiso circulatorio se administró adrenalina titulada, con mejoría progresiva de la ventilación y la perfusión. Se tomaron muestras seriadas de triptasa sérica y se indicó evaluación por alergología para identificación del agente desencadenante y planificación anestésica futura. Este reporte enfatiza un enfoque estructurado para el reconocimiento temprano, el descarte rápido de causas reversibles y la instauración oportuna de adrenalina cuando el cuadro sugiere anafilaxia.

Abstract

Perioperative bronchospasm is a time-critical emergency that may lead to severe hypoxemia, hypercapnia, abrupt increases in airway pressures and hemodynamic instability, particularly during induction and airway instrumentation. Its differential diagnosis is broad and includes mechanical causes (endotracheal tube or circuit obstruction), endobronchial intubation, aspiration, tension pneumothorax, cardiogenic or non-cardiogenic pulmonary edema, and bronchoconstriction due to airway hyperreactivity or perioperative anaphylaxis. We report a 56-year-old male scheduled for elective laparoscopic cholecystectomy under general anesthesia who developed severe bronchospasm immediately after tracheal intubation, with markedly elevated peak inspiratory pressures, reduced tidal volume, oxygen desaturation and a classic "shark-fin" capnogram. Concomitant hypotension occurred without obvious skin signs, prompting high clinical suspicion of perioperative anaphylaxis. Initial management included 100% oxygen, manual ventilation to assess compliance, systematic exclusion of mechanical problems, deepening anesthesia, inhaled bronchodilators and hemodynamic support. Because of incomplete response and circulatory compromise, titrated intravenous epinephrine was administered, resulting in progressive improvement of ventilation and perfusion. Serial serum tryptase samples were obtained and the patient was referred for allergy work-up to identify the culprit agent and plan safe future anesthesia. This case highlights a structured, reproducible approach to severe bronchospasm during induction, prioritizing early recognition, rapid identification of reversible causes and timely epinephrine when anaphylaxis is suspected.
